Go Online: Visit the DBPR's complaint portal at MyFloridaLicense.com.

Choose Complaint Type:
General Complaint: For issues with licensed professionals or businesses (e.g., contractors, real estate agents, cosmetologists).
Foodborne Illness: For suspected illnesses from food service establishments.
Unlicensed Activity: For individuals performing regulated work without a license.

Complete the Form:
Fill out the electronic form or download the Uniform Complaint Form (DBPR 0070).
Provide specific details, dates, names, and any supporting documents (receipts, photos, contracts).
Submit: Follow the instructions to submit electronically or mail the form and documents to the DBPR's Complaints Unit in Tallahassee.
